OhioRISE is Ohio's first ever integrated program to help children who have complex behavioral health and multi-system needs. OhioRISE (Resilience through Integrated Systems and Excellence) uses a system of care approach guided by the principals of a wraparound philosophy to provide care coordination.
Harbor serves as a Care Management Entity (CME) through OhioRISE, for children, youth, and their families in 11 counties, including Wood, Ottawa, Erie, Sandusky, Seneca, Wyandot, Hancock, Huron, Crawford, Marion, and Union.

Who will benefit:
Youth with behavioral health conditions that are also served by multiple other systems including child protection, juvenile justice, intellectual and developmental disabilities, and education. They also may be at risk of out-of-home placement or are returning to their families from out-of-home placement.
Who is eligible:
Children and youth age 0-20 who are eligible for Ohio Medicaid
And have one of the following:
- Require significant behavioral healthcare measured by the Ohio Child and Adolescent Needs and Strengths (CANS) assessment
- A recent in-patient hospital stay for mental illness or substance use disorder
- A recent in-patient stay in a Psychiatric Residential Treatment Facility (PRTF)
What is a CANS assessment?
A CANS assessment is a tool used to determine eligibility for OhioRISE. It gathers the youth's and family's story to understand their needs and strengths to determine the best ways to provide help. The CANS assessment is updated regularly to aid with planning ongoing care.
OhioRISE provides the following behavioral health services:
- Intensive and Moderate Care Coordination
- Intensive Home-Based Treatment (IHBT)
- Psychiatric Residential Treatment Facilities (PRTF)
- Behavioral health respite
- Flex funds to support implementing a care plan
- Mobile Response and Stabilization Services (MRSS)
- 1915(c) Home and Community Based Services (HCBS) Medicaid waiver that offers additional services for qualifying children and youth with serious emotional disturbances and functional impairments
Care Coordination
Care Coordinators have experience working with youth and families. They will help you find the specialized services and support that you need in your community. They use a wraparound philosophy with the youth and family at the center and the team wraps around them to provide services, supports, education and resources. This approach is guided by the family's views and ideas about what they need, their strengths, and professional support.
The team consists of the youth, family members/caregivers, care coordinators, care management entities (CME), and service providers.
