Community-Based Assessment
CBA Site Development This service is used to set up a CBA site if one does not exist, and to complete intake if a new referral.

CBA hourly shared coach
Our shared coach rate is used when our coach can be shared
between two consumers at the same time. All hours with two consumers
are billed at this special rate. If only one consumer shows for day,
the rate reverts to our standard CBA rate.

Vocational Evaluation
Vocational Evaluation
Service involves a high-caliber assessment of
aptitudes: abilities, vocational interests, work attitudes, work
personality, and transferable skills blended with medical,
psychological, social and economic data to determine realistic
vocational goals for the person. Includes an intensive interview,
broad-based testing, a computerized job matching process, and labor
market outlook information to specify job goals. Some examples of
instruments utilized for the assessment include: Self-Directed-Search,
Myers-Briggs Type Indicator, Career Aptitude Placement Survey, Wide
Range Achievement Test, Wonderlic Personnel Test, and work attitude and
motivation surveys. Additional tests may be used as needed to
individually tailor the assessment. A comprehensive report is provided
to the referral source within one week of the completion of the
evaluation. This service includes a follow up review session with
consumer and counselor.
Career Exploration Based
on counselor referral questions, this service may involve labor market
research, informational interviewing, and job shadowing to assist
individual in obtaining more information on specific occupations and

Vocational Evaluation Consultation Service
to be used in assisting consumers in processing information from
vocational evaluation and-or in career planning. Counselor may utilize
some vocational instruments during the session to further assist
consumer.
Personal Adjustment
Personal Adjustment Coaching A
job coach works with the individual to assist in learning new behaviors
or resolving barriers that are interfering with the person's ability to
successfully engage in work activities. Some of the issues the job
coach may tackle with the individual include: time management,
organizational skills, personal hygiene,
arranging child-care, resolving issues around criminal records, travel training, etc.
Adjustment to Disability Counseling
Counseling
that focuses on processing with an individual their thoughts and
feelings about their disability. Counselor works with person to develop
understanding of disability and to promote hope of living with the
disability versus being controlled by it.
Travel Training
A
job coach works with the individual to learn to ride the TARTA system
independently. Coach reviews TARTA routes, bus stops, fees (daily
fares, weekly passes, transfer fares, and reduced fare cards), how to
signal bus driver to stop, bus riding etiquette, and pre-trip planning.
Coach will also arrange to practice riding the bus with the individual
to specific destinations (i.e., employment site, doctors offices,
grocery store, etc.) until the person feels comfortable in riding the
bus on his/her own.
Budgeting Assistance
Individualized
support to assist consumer in developing a budget. Income and Expenses
are detailed in a spreadsheet for the consumer. Staff review with
consumer solutions to make sure income is covering expenses and discuss
how to eliminate non-necessary expenses. May include linkage with
non-profit credit counseling agency for long term support.
Anger Management (8 sessions - group format) A
behavioral approach to understanding and managing anger. This service
is provided in a group format and meets weekly for 1.5 hours for 8
weeks. The individual learns to identify their triggers and techniques
to better manage their anger. Certificates of completion are available
upon completion of the group.
Work Adjustment
Work Adjustment Site Development (Hourly Rate Available) Zone 1 - Lucas, Wood
Zone 2 - Ottawa, Sandusky, Fulton, Henry, Defiance, Williams, Paulding, Putnam, Van Wert, Allen, Hancock
Work Adjustment Weekly Rate Available (up to 20 hours) An
intensive focused program designed to help someone in improving
behaviors and attitudes to promote successful adjustment to work. Work
adjustment trainer utilizes information obtained from work adjustment
assessment to tailor program. Issues that could be dealt with in the
program include: accepting supervision, accepting and responding to
feedback, problem-solving on the job, communication skills with
co-workers, handling frustration effectively on the job, etc.
Work Adjustment Assessment A
pre-screening to identify work adjustment issues to develop an
individualized work adjustment program. A Certified Rehabilitation
Counselor meets individually with person for an intensive interview
session and administers work adjustment screening instruments to
identify work adjustment issues to be addressed in work environment.
Counselor receives a report with issues to target in a work adjustment
program.
Work Experience (Weekly & Monthly Rates available) - Up to 40 hours a week This
service can be used in two different manners. First, it can be used as
a working interview when the individual tries out a job for a specified
amount of time, demonstrating his/her work behaviors, to help the
employer in making a hiring decision. It can also be used in a
work hardening manner when the individual engages in some work
activities to build up their physical stamina, to get back into the
habit of keeping a work schedule and good work habits.
Employment Enhancement Program (Minimum of 4 participants needed)
Employment Enhancement Program with 48 hour CBA Program
for those experiencing neurological involvement (i.e., TBI, MS, severe
ADHD, CVA) with symptoms that are interfering with successful
employment. The consumer must be willing to work upon those goals set
by themselves, referral source, and instructors. Program is facilitated
by neuropsychologist, occupational therapist, and job coach with
specialized training in working with consumers with neurological
involvement. Purpose of program is to identify behaviors that are
interfering with successful employment and learning how to manage them
through practice in a supported environment. Participant has regular
weekly group meeting, and intensive follow along with neuropsychologist
and occupational therapist. Consumer must be attending a CBA site, job
site, or school several days a week. Service includes monthly staffing.
Work Readiness Assessment
This
service involves a clinical interview by licensed clinician to
determine if mental health diagnosis exists and administration of work
adjustment, self-esteem, and other work readiness inventories to
determine the individuals readiness to engage in employment services. A
summary report is created with recommendations. Service includes follow
up staffing.
Job Coaching
Starting a new job is exciting, challenging, and sometimes can be
difficult. A job coach can provide assistance in getting started in a
new job. The job coach provides individualized strategies for learning
job duties and successfully performing job duties. The job coach
utilizes a structured approach in breaking down job tasks into
trainable steps and uses a behavioral approach to teaching job tasks.
Job coaching can occur at the work site or off-site depending on
individual needs and preferences. The job coach can assist with
identifying and implementing possible job accommodations. Coaches help
with getting feedback from the individuals supervisor on work
performance and can assist person in learning the soft skills necessary
to maintaining employment.
Job Placement
Job Development A
job developer works with the person individually to assist the
individual in obtaining employment consistent with the individuals job
goal. Job developers are professionals who have an extensive knowledge
of the local labor market, have relationships with local employers, and
possess knowledge of what it takes to get a job. The developer provides
consultation on job search strategies, access to job search resources,
job leads, and assists with introducing the individual to potential
employers to help the person in getting landing a job offer. Zone 1 -

Job Development Follow Up Used
to support individual on the job, job developer meets with employer and
employee regularly to obtain feedback on work performance and resolve
employment issues to promote job retention for first 90 days of
employment.

Job Seeking Skills Training This
service involves working with a professional to prepare for a job
search. Activities include developing a resume, practice completing job
applications, mock interviewing and developing strategies to respond to
difficult interview questions (i.e., criminal record, history of
short-term employment, fired from past jobs, limited work history,
etc.), information on local job search resources and sources for job
leads, strategies for organizing your job search, and discussing issues
around disclosure of disability and identifying possible job
accommodations.

Job Development Follow Up Zone 2 - Ottawa, Sandusky, Fulton, Henry, Defiance, Williams, Paulding, Putnam, Van Wert, Allen, Hancock
Job Seeking Skills Training Group (minimum of 7 in group) Class
meets for 2 ½ hours twice a week over a 4 week period (20 hours of
class instruction). Curriculum includes instruction on: application
completion, resume development, cover letters, references, interview
strategies, networking techniques, cold calling, and training on
disability disclosure - negotiating accommodations.
Informational Interviewing This
service can be used to help a person obtain information on a specific
occupation. Staff schedule an appointment for the individual to meet
with an employer to interview the employer on specific occupations
(i.e., what qualifications the employer looks for in applicants,
company projections for hiring, wage range, etc.). The information
gathered during the informational interviewing process can help in
setting career goals and preparing for job interviews Zone 1 -

Job Shadowing Often
in exploring careers, reading or hearing about a job isnt enough. Job
shadowing is a service that allows the person to observe and interact
with someone performing a particular occupation. Through this process
the individual gains more in-depth information on the occupation and

Job Analysis
The
job analysis provides information on the work environment and specific
job functions. Information provided in a job analysis include:
production quotas, worksite cues, physical requirements, skills
required in performing the job, and work environment characteristics. Zone 1 -

Other
Financial Benefits Analysis
Service
to assist a person in obtaining information on how working would effect
their present financial situation. This information can help the person
in making an informed choice in choosing whether or not to pursue
employment. Staff gathers information on persons current expenses and
income (i.e., SSI, SSDI, Medicaid, Medicare, OWF, Foodstamps, Section
8, etc.) and examines how different employment scenarios will effect
financial situation to identify what scenarios will improve persons
financial situation. A report is generated that summarizes the
individuals benefit information.
Financial Benefits Advocacy
Individualized
support to assist consumers in resolving issues related to their
benefits. Service may include development of a PASS plan, assisting
consumers in reporting their benefits regularly to agencies,
implementing IRWEs, resolving wage garnishment issues, assisting
consumers in getting benefits reinstated, and resolving overpayments.
Rehabilitation Technology Services This
service encompasses assessing need for accommodations made to either
home or work environments, designing needed accommodations, ordering of
supplies and equipment to create accommodation, and implementing
accommodation.

Psychological Testing Fee
is per RSC Medical Fee Schedule. Psychological testing services
include administering test batteries for Disability Screening and
Screenings for Mental or Emotional Disorders. Individual tests or
individualized batteries can be utilized per referral source request.
Neuropsychological Evaluations Fee
is per RSC Medical Fee Schedule. This neuropsychological evaluation
utilizes neuropsychological tests with an emphasis upon applying those
results in the vocational setting. It addresses typical
neuropsychological functioning concerns such as cognitive strengths and
weaknesses, as well as best level and type of training, effect of the
injury upon vocational functioning, accommodations to consider, and job
characteristics to seek/avoid.
Learning Capacity Evaluations
Fee
is per RSC Medical Fee Schedule. The Psychological - LCE utilizes the
test instruments required by colleges and testing agencies (Educational
Testing Service, etc) in order to determine whether an individual meets
the requirements for accommodations for a Learning Disability or an
Attention-Deficit - Hyperactivity Disorder. Assessment for emotional -
personality status is possible as well, if requested. Interest testing
can be completed as well, if requested.
EQ Assessment The
Bar-On Emotional Quotient Inventory measures emotional intelligence in
five areas: Intra-personal skills, Interpersonal skills, Adaptability,
Stress Tolerance and General Mood. It is the first reliable and valid
measure of Emotional Intelligence, normed in North America, and is
based on over 17 years of research by Dr. Reuven Bar-On. It provides a
snapshot of a persons interpersonal abilities and resiliency to
handling change. The assessment includes an individual feedback
session, summary report, and strategies for improving lower score
areas. The feedback session also includes matching occupational
profiles based on the individuals scores (i.e., how does a persons
scores line up with a particular occupational area).
Short Term Individual Counseling Brief
solution focused counseling to assist person in working through an
issue that is impacting their ability to get or keep a job (5 8
sessions).

Sign Language Interpreting On-site American Sign Language (ASL) interpreting
Spanish Speaking Interpreting
Tutoring (Academic) Individualized academic tutoring provided by certified teacher is available in Math, English, and Science.
Occupational Skill Training Program This
service involves 80 hours of paid employment training by employers in
specific occupations. Career Connections staff stay in regular contact
with employer and consumer through the training. A written report and
monthly staffing are included as part of this service. Consumers obtain
hands on experience and receive individualized training from employers
in community employment sites. This service can be further customized
by adding individualized job coaching as needed. Current occupations
include: Alterations - Seamstress, Bank Teller, Food Service, Cashier,
Janitorial, Child Care, Auto Detailing, Dry Cleaning, Customer Service,
Housekeeping, Laundry, Dog Bather - Groomers Assistant
Clerical Internships A
15 hour per week paid clerical internship. Service can be used to
assist consumers with gaining clerical experience while preparing or
engaging in a job search.
Support Services
The
intent of support services are when staff are requested to provide more
of a case management type service in linking consumers with the
supports necessary to obtaining and maintaining employment consistent
with the consumers individualized employment goals. Service can be
requested when the consumer does not qualify for case management
through existing agencies.
